The first is a failure of the gun to complete the cycle of operations. In a semi-automatic pistol this is usually a failure to feed, or a failure to eject/extract, or a dud round. Changing mags, total strip clean and lube of the gun, bending the extractor, bending mag feed lips, trying different ammo. Ive seen them all. After the initial 50-200rd break in period a gun should work 99.9% of the time.
